What is an Advance Statement?
An Advance Statement is a form that lets you record the care you want in the future. It is a record of your wishes, feelings, beliefs and values, which can be used if you later become unwell and need care or medical treatment. It is not legally binding, however what you write is still important because it must be taken into account when someone is making a decision for you.

You can download an Advance Statement template form [EXTERNAL PDF LINK] to print and complete by hand. Once you have finished, you need to share a copy with your GP and those close to you.
